Option 1: Access Records Through the ChiroHD Patient App
Fastest Option — Self-Serve Access
If you are an existing patient, you may already have access to your records through the ChiroHD patient app. The app allows you to view your appointment history, documents, and X-rays directly from your phone.
If you previously created a ChiroHD account through our office, log in with your existing credentials. If you need help accessing your account, contact us at [email protected].
Option 2: Submit a Records Request
If you need a complete copy of your records, need records transferred to a new healthcare provider, or are unable to access the ChiroHD app, you may submit a formal records request.
How to Request Your Records
- Download and complete the Authorization for Release of Medical Records form (see below).
- Submit the completed form by one of the following methods:

- Phone: Call 720-702-4727 to request records verbally (identity verification required)
- Receive your records within 30 days of our receipt of your completed request.
Print, complete, sign, and return by email or mail.
What to Include in Your Request
Whether you use the authorization form or submit a written request, your request must include:
- Your full legal name (as it appears in our records)
- Your date of birth
- A description of the records you are requesting (e.g., complete record, treatment notes only, X-rays)
- The name and address of the receiving provider (if transferring records)
- Your signature (or the signature of your legal representative)
- The purpose of the request (e.g., continuation of care, personal use)
We may contact you to verify your identity before releasing records, as required by HIPAA.
Timeline & Fees
Processing Time
We will process your request within 30 days of receiving your completed authorization. If additional time is needed (for example, if records are archived), we will notify you in writing with the reason for the delay and a date by which your records will be provided. The maximum processing time is 60 days from receipt of your request.
Cost
There is no charge for requesting your medical records from Back In A Flash Chiropractic & Massage. We will provide copies of your records at no cost to you.
Record Retention
Your medical records will be securely retained for a minimum of seven (7) years after your last date of treatment, in accordance with Colorado state law and the Colorado Board of Chiropractic Examiners regulations. For minor patients, records will be retained for seven years after the patient reaches age 18, or seven years after the last date of treatment, whichever is later.
All records are stored electronically in a HIPAA-compliant system and are protected by industry-standard security measures.
Your Rights Under HIPAA
As a patient, you have rights regarding your health information under the Health Insurance Portability and Accountability Act (HIPAA). These rights include:
You have the right to see and receive a copy of your health records maintained by our practice.
You may request that incorrect or incomplete information in your record be amended.
You may request a list of certain disclosures we have made of your health information.
You may request restrictions on certain uses and disclosures of your information.
